We created our membership plans with the optimum health of our patients in mind, and have made an effort to do away with many of the pesky restrictions that dental insurance tends to implement. With every membership plan, you will experience dental care with no yearly maximums, deductibles, or insurance claims. There are also no limitations for pre-existing conditions, and no waiting period, which means you’ll get the dentistry you need, today.

THIS PLAN IS NOT INSURANCE AND IS NOT A HEALTH BENEFIT PLAN.
Membership plans are not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services. Rather, Membership plans are direct payment arrangements between plan members and Providers for services, products and care provided. Participating providers have individually contracted to provide discounts to plan members. This plan does not make payments to providers for services you receive. Payment for treatment not included in the membership plan is due at the time of service. You are obligated to pay for all services received. The savings offered in the membership plan can not be combined with any other offers or discounts. Plan Forward, Inc. charges Providers for the use of the Plan Forward platform and services. Discounts available under this plan will vary depending on the type of provider and service received.
This plan runs for 12 months and includes an automatic renewal feature. You will be notified 30 and 60 days prior to your renewal date of any changes to your membership fees, and may opt out at any time. You may cancel at any time by contacting your Provider’s office, or reaching out to Plan Forward at 1-833-752-7526 or at care@planforward.io. Upon cancellation within the first 30 days of the current annual term, you will receive a full refund of periodic charges paid, minus the value of any services already received. Thereafter, refunds will be prorated for the unused portion of the term.
